Under the supervision of the Assistant Director and/or Director, the Prevention Specialist will go to the communities of Pomona to facilitate all prevention programming components.

Specific Responsibilities: 

• Provide ongoing prevention services, including alternative activities, information dissemination, problem identification, and referrals.
  
• Co-facilitate groups.
  
• Disseminate prevention information and materials.
  
• Carry out general prevention/intervention activities, referrals to drug treatment, public health, HIV and social service agencies.
  
• Document information on each contact and maintain written records and reports.
  
• Assist the program in developing and documenting and culturally sensitive understanding of the target population.
  
• Enhance community-wide awareness of the program, its services, and research.
  
• Provide direct services and transportation based on client needs.
  
• Assist in training community network staff representatives.
  
• Other duties as assigned. 

Qualifications:

High School Diploma or GED. Two years experience working with women and/or substance abuse populations and have access to target neighborhoods.  Must have knowledge and credibility in target populations. If a recovering person, verified drug-free status with strong recovery program. Good writing and verbal communication skills. Willingness to work evenings and weekends.  The Substance Abuse Counselor provides intakes, counseling and education to outpatient and/or day treatment clients.  In addition, they also provide case management services to ensure community linkage, especially assisting perinatal clients with support needed for parenting.  Demonstrates the ability to obtain and interpret information in terms of the client’s needs, documents knowledge of client’s growth and progress and demonstrates an understanding of the range of treatment needed by clients served.  Must enjoy working as a member of a team in a cooperative effort and be able to exercise creative and independent judgment to solve problems.  Expected to perform their duties in alignment with the vision and values of PROTOTYPES.

Specific Responsibilities: 

• Conducts initial assessment and intake evaluation for the formulation of treatment plans.
  
• Provides group, family and individual counseling.
  
• Ensures both internal and collateral services are coordinated.
  
• Provides court reports or other reports as requested. 
  
• Works closely with CASC Assessors.
  
• Assists client in developing appropriate treatment goals and makes revisions when necessary.
  
• Provides substance abuse education to clients and to community groups.
  
• Participates in team meetings to provide information and make recommendations for treatment, discharge planning and client management.
  
• Participates in various meetings, including case conferences, group supervision, staff meeting and daily update.
  
• Participates in program planning.
  
• Continues professional development through participation in continuing education, in-service training and professional workshops to maintain current knowledge and broaden expertise.
  
• Maintains client records in a timely and appropriate manner and in accordance with County and Federal guidelines.  Handwriting must be clear and legible.
  
• Ensures that all documentation is completed within 72 hours of contact. 
  
• Documents client’s progress and reviews treatment plans routinely.
  
• Complies with all policies and procedures.
  
• Performs other related duties as assigned.

Qualifications:

 If recovering, minimum of two years sobriety is required. AA degree in Human Services with two (2) years experience or three (3) to five (5) years equivalent experience in the field of substance abuse treatment.  Fewer years experience may be accepted for an Entry Level Counselor in training.  Experience with culturally diverse substance abuse populations with co-existing mental health diagnosis.  Ability to understand and interpret information, follow instructions and manage workflow.  Performs a flexible schedule that may include evenings and weekends.  Strong interpersonal skills.  Ability to work effectively with people in a constructive manner within a community-based treatment setting.  CPR and First Aid Certifications are required.  CADAC/CAC/CAS Certification.  Ability to communicate in a clear, precise and professional manner.  Valid driver’s license, registration and insurance required.

Responsible for all clinical aspects of all Residential Programs in Pomona. Supervise MSW/MFTI staff collecting hours for licensure and substance abuse counselors on clinical issues. Provide mental health treatment services to clients within residential programs. Oversee development of new programs. Responsible for the design, development and implementation of clinical policies and procedures in conjunction and consultation with the Director of Residential Housing and others. Administrative tasks as assigned.

Specific Responsibilities: 

• Provide clinical oversight to all residential programs in Pomona.
• Provide clinical supervision to unlicensed clinical staff; including therapists and substance abuse counselors.
• Ensure compliance with SAPC, HIPAA and other funder and legal documentation standards.
• Oversee and direct the clinical treatment program at PWC including policies and procedures for intake, treatment and discharge.
• Monitor therapeutic activities on site, provide relevant training to facility staff and provide special assessments and crisis intervention as needed.
• Convene and facilitate regular, ongoing mental health team meetings and case conferences, and participate in facility management, staff and other meetings.
• Develop and maintain contacts with community providers, coordinating agencies and government agencies, implementing a network of provider meetings in support of the program and representing PROTOTYPES in appropriate meetings and projects.
• Identify training needs and opportunities for staff; provide training as needed and appropriate.
• Provide assistance with administrative duties as assigned.
• Assist in crisis and symptom management.
• Interface with multidisciplinary team in treatment planning.
• Facilitation of the weekly treatment team meetings and supervise team to ensure that clients are receiving necessary services.
• Provide after hours on call emergency coverage.
• Arrange work schedule in accordance with the agency’s needs.
• Attend meetings as assigned.
• Comply with the agency’s policies and procedures.
• Perform other duties as assigned.

Qualifications:

Licensed Clinical Social Worker in California for a minimum of 2 years. Experience with providing clinical supervision. Strong clinical skills working with adults and children. Background in substance abuse services working in integrated systems of care. Strong understanding of working within a multidisciplinary team and in a collaborative model. Sound organizational skills required.
