Position Title:  Counselor/Case Manager

Reports To: Counselor/Case Manager Supervisor

Classification: Non-exempt

Worksite: PROTOTYPES Women's Center, Pomona

Job Summary: 
Responsible for facilitating development toward cessation of substance use while cultivating client's parenting, living/recovery skills to achieve their highest potential.

Duties & Responsibilities: 
1. Interview clients, reviews records, and confers with other team professionals to evaluate the condition of client.

2. Assists client in formulating a treatment plan at time of entry and makes revisions as necessary. .Evaluates treatment plans at stated intervals.

3. Provides individual and group counseling. Group curriculum development.

4. Timely upkeep and maintenance of chart documentation.

5. Assessment of mental health disorders and referrals for mental health follow-up.

6. Serve as liaison to collaborate with various social service agencies and legal systems to maintain client care (including letter and progress report writing in a timely manner).

7. Participate in staff meetings, program planning, team meetings, and in-service trainings as scheduled.

8. Develop client exit plans and facilitation of healthy transition back into the community.

9. Perform other duties as assigned.

Qualifications: 
High school diploma or G.E.D. Demonstrate therapeutic skills through prior training or work experience. Minimum of two year experience working with mentally ill population and/or substance abusing population with direct client contact. Completion of state certification process or registered and actively pursuing state certification. Valid California Class "B" driver's license. Willingness to work and learn in a community-based setting emphasizing integration of alcohol, drug, and mental health treatment concepts. Willingness to work evening and weekend hours.
