Position Title:  Full Time Case Manager

Position Location:  Pasadena Adult Outpatient Mental Health Clinic and Adult Full Service Partnership

Responsibilities:
· Provide case management services to adults with severe and persistent mental challenges

· Lead skills development groups

· Provide crisis and symptom management

· Document services in accordance with Agency Standards

· Complete documentation in a timely manner

· Maintain all client information in a confidential manner

Minimum Qualifications: 

· Bachelor’s degree in mental health related field

· Some experience working with co-occurring disorders

· Knowledge of DMH documentation standards preferred
