Our online risk assessment can help determine if you should contact us for a more in-depth assessment. The online questionnaire consists of only nine "yes" or" no" questions and does not require you to enter your name or contact information. Your responses will be kept confidential and will ultimately help us determine your needs.

The PROTOTYPES’ Community Assessment Service Centers (CASC) were established to help residents of the San Gabriel Valley access needed social services through local partner agencies. Currently, there are three sites located in El Monte, Pomona and Pasadena.

 

At each location, a trained CASC professional will administer a standardized assessment in order to evaluate the level of care required for each client. Upon completion and analysis of the assessment results, the CASC staff members will make a referral to a program best equipped to provide the necessary care.
