The Certified Substance Abuse Counselor/Case Manager is the coordinator of community and client services for participants in Prototypes’ programs. This can include outpatient or residential services in areas of domestic violence, substance abuse and issues of mental health. The Certified Substance Abuse Counselor/Case Manager is responsible for the assessment of need and implementation of care plans. It is usual for clients to have ongoing support needs in areas such as housing, employment, social relationships, and community participation. The Certified Substance Abuse Counselor/Case Manager will work closely with other case managers, counselors, and other health specialists to coordinate and monitor client issues. Often, the Certified Substance Abuse Counselor works in a group environment.
