Case Management

In all of Prototypes’ programs, case managers work with clients to create an individualized treatment plan that also includes a plan for after treatment.

This aftercare plan is critical to ensure the continued recovery of clients and support for their families. Case managers provide assessment and treatment planning, link clients to important resources in the community, monitor progress and advocate for the client. They are critical in supporting a client’s decision making and help remove any barriers to treatment or continued recovery. A strong working partnership is critical as clients transition among programs and levels of care.
